Cranberry doesn't work the way many people assume. It isn't about acidifying urine or “cleansing” the system — the real action comes from a group of plant compounds called proanthocyanidins, or PACs.
The anti-adhesion effect
Cranberries are unusually rich in A-type PACs. Laboratory and clinical research shows these compounds interfere with the ability of uropathogenic E. coli to stick to the cells lining the urinary tract. If bacteria can't adhere, they're far more likely to be flushed away in normal urine flow before they can multiply and cause an infection.
The dose-response study
A frequently cited 2010 multicentre, randomized, double-blind study published in BMC Infectious Diseases tested this directly. Participants took cranberry powder standardized to different PAC doses, and researchers then measured how well their urine prevented E. coli from adhering. The result was dose-dependent: higher PAC intake produced stronger anti-adhesion activity in the urine, with a meaningful effect observed around 36 mg of PACs per day.
Why juice often falls short
This is the crux of why standardized extracts matter. PAC content varies enormously between cranberry products, and juice is both dilute and high in sugar. A concentrated extract delivers a consistent, research-relevant PAC dose in a single capsule.
What this means for you
Understanding the mechanism helps set expectations: cranberry PACs are a preventive tool that makes it harder for bacteria to take hold — not a cure for an active infection.
