D-Mannose is a simple sugar that has become one of the most talked-about natural options for women who deal with recurring urinary tract infections. Here's what the clinical trials actually show.
The landmark randomized trial
The most-cited study is a 2014 randomized controlled trial published in World Journal of Urology. Researchers followed 308 women with a history of recurrent UTIs and assigned them to one of three groups: 2 grams of D-mannose powder daily, the antibiotic nitrofurantoin, or no preventive treatment.
Over six months, about 15% of women taking D-mannose had a recurrent UTI, compared with roughly 20% on the antibiotic and about 60% in the no-treatment group. In other words, D-mannose performed comparably to a standard preventive antibiotic — with notably fewer side effects reported.
What the wider evidence says
A 2020 systematic review and meta-analysis in the American Journal of Obstetrics & Gynecology concluded that D-mannose appears to reduce the risk of recurrent UTIs compared with placebo, while calling for larger trials to confirm the effect. A 2021 systematic review in European Urology Focus reached a similar, cautiously supportive conclusion.
How it works
Most UTIs are caused by E. coli bacteria that use tiny finger-like projections (FimH adhesins) to grip the bladder wall. D-mannose is thought to act as a decoy: bacteria bind to the mannose instead of the bladder lining, so they're carried out in the urine rather than taking hold.
