Ask most men whether they think low testosterone is common, and you’ll get a shrug. It’s something that happens to other people — older men, men with underlying conditions. Not me. Not yet.
The numbers disagree.
Testosterone deficiency is far more prevalent than most Canadian men realize, it begins earlier than most expect, and new research suggests it’s getting more common across generations — independent of age. Understanding the true scope of the problem is the first step toward addressing it.
The Canadian Data
According to the Canadian Urological Association’s evidence-based guideline on testosterone deficiency, the crude Canadian prevalence of biochemical testosterone deficiency is approximately 25% among men aged 40 to 62 years. One in four. That figure climbs steadily with age: prevalence is estimated at 4–12% in men aged 50–59, 9–23% in men aged 60–69, and continues rising through the seventies and beyond.
The Massachusetts Male Aging Study, one of the most cited longitudinal studies of male hormonal health, estimated the prevalence of low testosterone at 25.3% among men aged 40 to 70 using blood-based cutoffs alone. The American Journal of Medicine published findings placing testosterone deficiency at approximately 30% of men aged 40–79, with prevalence strongly correlated with aging and common comorbidities including obesity, diabetes, and hypertension.
These aren’t fringe numbers. They describe a condition affecting roughly one in four to one in three middle-aged and older men.
It Starts Earlier Than You Think
Testosterone levels begin declining around age 30, at a rate of approximately 1–2% per year on average. By the time a man reaches his 40s, that cumulative decline can be clinically meaningful — particularly when compounded by lifestyle factors, chronic stress, or metabolic conditions.
Perhaps more surprising is that low testosterone is increasingly being documented in younger men. Research using National Health and Nutrition Examination Survey (NHANES) data found that serum testosterone levels have been declining across successive generations of young adult males in the United States. A study of over 100,000 Israeli men tested between 2006 and 2019 reported a significant, age-independent drop in total testosterone across all adult age groups. A Swedish study comparing population samples from 1995 versus 2008 found that 2008 men had significantly lower free testosterone than same-aged men had thirteen years earlier.
In short: the decline in testosterone is not just about individual aging. It is a population-level trend, and it is accelerating.
Why Is This Happening?
Researchers describe the causes of this intergenerational decline as multifactorial and synergistic. The leading contributors include:
- Rising rates of obesity: Adipose tissue converts testosterone to estrogen via the aromatase enzyme. As body fat increases, testosterone decreases.
- Sedentary lifestyles: Physical inactivity reduces both the stimulus for testosterone production and the metabolic health that supports it.
- Chronic psychological stress: Elevated cortisol directly suppresses the hypothalamic-pituitary-gonadal (HPG) axis, the hormonal pathway governing testosterone production.
- Poor sleep: Testosterone production is tightly coupled to sleep, particularly REM sleep. Chronic sleep deprivation measurably suppresses testosterone levels.
- Exposure to endocrine-disrupting chemicals (EDCs): Phthalates, BPA, and microplastics have been identified as contributing to hormonal disruption at the population level.
- Diabetes and metabolic syndrome: The CUA guideline notes that 33% of men with diabetes have hypogonadism. The relationship between metabolic health and testosterone is bidirectional and reinforcing.
No single factor accounts for the full picture. The evidence points to a cumulative burden — a modern lifestyle that compounds hormonal stress across multiple pathways simultaneously.
The Health Consequences Go Beyond Libido
The most commonly recognized symptoms of low testosterone — reduced libido, erectile dysfunction, fatigue, mood changes — are real, but they represent only part of the clinical picture. Research has linked testosterone deficiency to:
- Increased cardiovascular mortality: men in the lowest quartile of testosterone carry a 40% increased likelihood of 20-year all-cause mortality
- Loss of lean muscle mass and increased visceral adiposity
- Insulin resistance and progression toward type 2 diabetes
- Reduced bone mineral density and elevated fracture risk
- Cognitive decline and mood disturbances
- Reduced quality of life across multiple domains
This is not a condition to dismiss as cosmetic or age-related. The downstream health consequences of untreated testosterone deficiency are substantive.
The Diagnosis Gap
Despite the prevalence of testosterone deficiency, most men with low testosterone go undiagnosed. Clinical testing is not routine. Symptoms — fatigue, low drive, changes in body composition — are often attributed to stress or normal aging without a blood draw. And when testing does occur, the standard cutoff of 300 ng/dL may itself be inappropriately low; emerging evidence suggests a threshold of 400 ng/dL may be more appropriate for men under 45.
If you’re over 40 and experiencing any combination of persistent fatigue, reduced motivation, difficulty building or maintaining muscle, changes in mood or focus, or declining libido, a total testosterone test — drawn between 7am and 11am when levels are highest — is a reasonable first step. The data suggests the results may surprise you.
What Can Be Done
The spectrum of options for men with low testosterone runs from lifestyle interventions to prescription testosterone replacement therapy, with licensed natural health products occupying a meaningful position in between. The right path depends on the severity of deficiency, the presence of symptoms, underlying health status, and individual goals.
What the evidence makes clear is that doing nothing — dismissing symptoms as inevitable, declining to test, assuming low testosterone is someone else’s problem — carries its own set of consequences. The numbers on prevalence are not theoretical. Statistically, one in four men reading this article has a testosterone level below the clinical threshold.
The question is whether they know it.
