Decades of epidemiological research and an increasing number of controlled studies have identified specific dietary patterns and foods associated with lower rates of BPH, chronic prostatitis, and prostate cancer. This is not alternative medicine — the dietary data for prostate health is as strong as it is for cardiovascular disease.
The Diet-Prostate Connection
Two broad dietary patterns emerge consistently from prostate health research. The Western diet — high in red meat, processed foods, refined carbohydrates, and saturated fats — is associated with higher rates of BPH progression and prostate cancer risk. The Mediterranean dietary pattern — rich in vegetables, legumes, fish, olive oil, and whole grains — is consistently associated with lower prostate disease burden.

1. Tomatoes (Lycopene)
Tomatoes are the richest dietary source of lycopene, a carotenoid antioxidant that accumulates in prostate tissue at concentrations several times higher than in the blood. Multiple epidemiological studies and a meta-analysis published in Cancer Epidemiology, Biomarkers and Prevention found an inverse association between lycopene intake and both BPH and prostate cancer risk. Cooked tomatoes (tomato paste, sauce) have higher bioavailable lycopene than raw tomatoes due to heat breaking down cell walls. Pumpkin seeds are one of the richest plant sources of phytosterols, particularly beta-sitosterol, which has been studied in multiple RCTs for BPH symptom relief. They are also an excellent source of zinc, an essential mineral concentrated in the prostate gland. A German observational study found that regular pumpkin seed consumption was associated with reduced BPH symptom scores and improved quality of life in men with lower urinary tract symptoms. 7. Walnuts and Brazil Nuts (Selenium, Healthy Fats)
Brazil nuts are the most concentrated dietary source of selenium, a trace mineral with antioxidant activity in prostate tissue. While the SELECT trial (2011) found that selenium supplementation in high-dose isolated form did not reduce prostate cancer risk, dietary selenium from food sources remains associated with prostate health in epidemiological studies. Walnuts provide alpha-linolenic acid (ALA), a plant-based omega-3, along with polyphenols with anti-inflammatory activity. Quercetin is the active compound in several clinical trials for CP/CPPS, demonstrating significant NIH-CPSI score improvements versus placebo. Dietary quercetin intake provides background support; therapeutic doses for CP/CPPS require supplementation.
Foods to Limit or Avoid
- Processed red meat: High intake of processed meats (sausage, bacon, deli meats) is consistently associated with increased prostate cancer risk in large cohort studies, likely due to heterocyclic amines formed during high-temperature cooking

- Excessive caffeine and spicy foods: Can irritate the bladder and worsen urinary urgency and frequency in men with existing BPH or prostatitis
The Dietary Pattern That Matters Most
Individual foods matter, but the overall dietary pattern matters more. A prospective study of nearly 4,000 men by Parsons et al. published in the Journal of Urology found that adherence to a Mediterranean-style dietary pattern was independently associated with reduced BPH progression and lower IPSS scores at follow-up, even after adjusting for age, BMI, and physical activity. The Mediterranean diet's combination of anti-inflammatory foods, antioxidants, and healthy fats appears to create a hormonal and inflammatory environment that is genuinely protective for the prostate.
